Prevea Health is an integrated multi-specialty group. With primary care providers and exceptional specialists, we offer services that can’t be found anywhere else in Northeast Wisconsin. We also provide our high-quality health care services in Western Wisconsin’s Chippewa Valley region.

We partner with six hospitals: HSHS St. Vincent Hospital and HSHS St. Mary’s Hospital Medical Center in Green Bay, HSHS St. Nicholas Hospital in Sheboygan, HSHS St. Clare Memorial Hospital in Oconto Falls, HSHS Sacred Heart Hospital in Eau Claire and HSHS St. Joseph’s Hospital in Chippewa Falls. We share one combined electronic medical record with our hospital partners, so wherever a patient is seen–in one of our health centers or hospitals—his or her complete medical record will be readily available to all treating providers. This ensures the highest continuity of integrated care.
